On a second photocopied page, possibly the reverse of the certificate, carrying the title "UNITED STATES STANDARD CERTIFICATE OF DEATH," in a space entitled "ADDITIONAL SPACE FOR FURTHER STATEMENTS BY PHYSICIAN," was written "Had her right breast removed last fall for carcinoma. At this time there were [unreadable word] in glands of milk & no doubt internal organs, the exact loctoin [sic--location?] is not known as no autopsy was made. [unreadable signature]."
