The death certificate date, 1845, is consistent with age 5 shown in the 1850 census, and agrees with the year shown in the 1900 census, so I conclude that the gravestone inscription showing the year as 1846 is in error.

Martha W. Bacon, Martha W., Certificate of Death, shows date, as the 4th, and address, with the informant being Mr. Cooper Turner, her son-in-law, with the same address.
Matthew Lyddall & Martha W. Bacon family monument, Elmwood Cemetery, shows date, as the 6th. The date of the 4th from her death certificate seems more compelling, particularly with the cemetery records showing interment on the 5th.
